Composition
Clomipramine HCl 25 mg & 50 mg.

Indications
1. It is effective in the relief of major depressive episodes, bipolar disorder and atypical depression.
2. It relieves symptoms of obsessive compulsive
disorders and phobias.
3. It is used in adjunctive treatment of cataplexy associated with narcolepsy.
4. It is effective therapeutic agent for premature ejacula tion.

Dosage
For major depressive episodes, bipolar disorder and atypical depression:
- For adults: Initially 25 mg three times a day, increased gradually to 100 mg during the first 2 weeks. The dosage may be further increased up to 250 mg daily. Usual maintenance dose 30 - 50 mg daily (severe cases 50 -100 mg daily).
- For children and adolescents: Initially 25 mg once a day, the dose being increased as needed and well tolerated up to 100 mg a day or 3 mg/kg of body weight or to a maximum of 200 mg whichever is less.
- For premature ejaculation: 25 - 50 mg/day or 25 mg 4 to 24 hours prior to intercourse.

Mechanism of action
- Clomipramine has several pharmacological actions such as anti-depressant, antiobsessive, antipanic, antineuralgic, anticataplactic and antibulimic.
- Mechanism of action in premature ejaculation: Supranil® activates 5-HT2C receptor, therefore, changes the set point to a higher level, leading to a delay in ejaculation.
Supranil® elevates the penile sensory threshold. Supranil also inhibits the reuptake of noradrenaline.
